In commitment to the patients we serve every day, Saint Vincent Hospital has made it a priority to establish a culture of transparency that encourages “blameless” reporting of all unanticipated events to regulatory agencies, regardless of the outcome. We share the Department of Public Health’s goal of using the process of aggressively and transparently reporting and analyzing events so we can participate collaboratively in the prevention and to the extent possible, elimination of these events in the future.

Our aggressive reporting philosophy has led to systemic changes to hospital policies and protocols, open discussions and state-of-the art professional development programs among hospital administrators and staff, and new capital investment – all aimed at achieving optimal patient safety.
