Saint Vincent Hospital Selected for Transition Program

The Massachusetts Hospital Association (MHA) has announced that six Central Massachusetts hospitals have been selected to participate in an affordable care act program aimed at improving patient transitions between health care settings and reducing admissions.

Clinton Hospital, HealthAlliance Hospital, Marlborough Hospital, MetroWest Medical Center, Saint Vincent Hospital and UMass Memorial Medical Center will partner with Worcester-based Elder Services and Bay Path Elder Services in the program.  Wing Memorial Hospital in Palmer, a UMass hospital, will also participate.

The Massachusetts Hospital Association provided technical assistance to the hospitals in applying for program funding.
