Saint Vincent Hospital Community Benefits Program
Request for Proposals
Saint Vincent Hospital’s Community Benefits Program (SVH-CBP) is seeking grant proposals for health-related initiatives that serve vulnerable populations in the Greater Worcester area. SVH-CBP supports causes that affect the health of our communities and funds qualified 501 (c) 3 organizations that meet the eligibility criteria and operate in the Hospital’s focus areas. While all types of health-related initiatives may apply for funding, this year the Committee is encouraging applications that are geared toward the prevention of disease, facilitating access to healthcare, as well as other initiatives such as preventing infant mortality, smoking cessation, preventing and overcoming obesity, and dental care and oral health.
Requests for funding should include a completed grant application--available online at www.stvincenthospital.com--along with supporting documentation, which should include:
· an explanation of how the request meets the mission of the Saint Vincent Hospital Community Benefits Program
· the need to be addressed and the number of people to be served
· the geographical area of service
· the strategies that will be used to meet the stated healthcare needs
· a method of evaluation.
Applicants are eligible to apply for a grant between March 1 and April 14, 2008 and for amounts up to, and including, $5,000. The SVH-CBP will consider grant applications which most nearly match their mission. Saint Vincent Hospital follows the ideals and traditions of Catholic healthcare.
On or before June 1, 2008 notifications will be made to those programs or services which have been awarded funding.
