Diagnostic Cerebral Angiography
Overview
Diagnostic cerebral angiography is a catheter-based imaging procedure that gives highly detailed pictures of the blood vessels of the brain, neck, and sometimes spinal circulation. Even in an era of excellent CTA and MRA, cerebral angiography remains one of the most precise ways to define aneurysms, AVMs, dural AV fistulas, carotid disease, venous outflow problems, and other complex neurovascular disorders.
For many patients, angiography is not the treatment itself but the procedure that allows the treatment plan to be made with the greatest accuracy.
Why angiography is performed
A cerebral angiogram may be recommended to:
- Clarify a possible aneurysm or better define its neck and branches.
- Evaluate AVMs or dural AV fistulas in detail.
- Assess carotid or intracranial stenosis.
- Investigate pulsatile tinnitus or venous sinus pathology.
- Confirm whether treatment is needed and which treatment is safest.
How the procedure works
A small catheter is placed into an artery, usually through the wrist or groin, and carefully guided under X-ray into the vessels supplying the brain. Contrast dye is injected while high-speed images are obtained.
The procedure is minimally invasive, but it is still an invasive vascular test. That distinction matters: it avoids open surgery, yet it still requires precision, sterility, contrast administration, and procedural risk counseling.
What patients may experience
Patients may feel:
- A brief warm sensation with contrast injection.
- Pressure at the access site.
- Mild soreness afterward.
Depending on the case, the procedure may be performed with local anesthesia, conscious sedation, or a deeper level of anesthesia.

- Highest level of anatomic detail in many cerebrovascular disorders.
- Ability to see blood flow dynamics in real time.
- Better treatment selection.
- In some cases, the possibility of transitioning from diagnosis to therapy in a coordinated neurovascular pathway.
Risks
Potential risks include:
- Stroke or transient neurologic symptoms.
- Bleeding or bruising at the access site.
- Arterial injury.
- Contrast reaction.
- Kidney stress in susceptible patients.
These risks are usually very low in experienced hands, but they are important enough that angiography should be performed only when the expected value is meaningful.

A cerebral angiogram is often recommended when noninvasive imaging has raised an important question that still needs a more definitive answer. In many cases, it is the study that turns uncertainty into a precise, individualized treatment plan.
