Overview

A dural arteriovenous fistula, often abbreviated dAVF, is an abnormal connection between arteries and veins in the dura, the covering around the brain. Unlike a classic brain AVM, a dAVF typically represents abnormal shunting into a dural venous sinus or cortical vein rather than a compact tangle within brain tissue itself.

Dural fistulas vary from relatively low-risk lesions to dangerous high-grade lesions with cortical venous reflux and elevated risk of neurologic injury or hemorrhage. The natural history depends heavily on venous drainage pattern, which is why complete angiographic evaluation is so important.

Common symptoms

Symptoms may include:

  • Pulsatile tinnitus, or hearing a heartbeat-like sound in one ear or both ears.
  • Eye symptoms if the lesion affects venous drainage near the cavernous sinus.
  • Neurologic deficits.
  • Intracranial hemorrhage in more aggressive lesions.

Why venous drainage matters

Low-grade fistulas without retrograde cortical venous drainage may sometimes be managed more conservatively, while higher-grade fistulas generally require treatment because the presence of cortical venous reflux is associated with greater risk.

How dAVFs are diagnosed

Initial evaluation may include CTA, MRA, or MRI, but diagnostic cerebral angiography is often essential because it defines the feeding arteries, venous outflow pattern, fistula location, and treatment options in detail.

Treatment goal

The treatment goal is to close the abnormal connection and normalize venous drainage while protecting normal brain circulation. Endovascular therapy is frequently the primary treatment strategy, though surgery or stereotactic radiosurgery may remain important in selected cases.

Treatment option 1: Endovascular embolization

Endovascular embolization is the most common treatment pathway for many dural AV fistulas. Through a catheter-based, minimally invasive approach, embolic material or coils may be used to close the abnormal connection, either from the arterial side, the venous side, or both depending on the anatomy.

In some sinus-based fistulas, the venous pouch or involved sinus is packed with coils to disconnect the lesion. In others, liquid embolic agents are used to penetrate and close the fistulous network.

Potential benefits include:

  • No craniotomy in many cases.
  • Ability to target the exact fistulous connection.
  • Often rapid symptom improvement, especially when pulsatile tinnitus is caused by the fistula.

Treatment option 2: Surgery

Surgery remains an important option when the endovascular route is not feasible, is incomplete, or carries excessive risk. Microsurgical disconnection may be especially useful for selected lesions with accessible venous drainage anatomy.

Treatment option 3: Radiosurgery

Radiosurgery may be considered in selected dAVFs, particularly when the lesion is not ideal for open or endovascular disconnection. Because closure after radiosurgery is delayed, it is generally not the preferred sole therapy for high-risk lesions needing immediate protection.

Follow-up after treatment

Even after an apparently successful treatment, follow-up angiography is often needed to confirm durable occlusion and ensure the fistula has not recurred or recruited new arterial supply.

For patients and families

A dural AV fistula is often very treatable, and many patients improve substantially after the abnormal connection is closed. The key issue is not simply the diagnosis itself, but the fistula’s venous drainage pattern and risk profile. That is what determines whether observation is reasonable or treatment should move forward more urgently.
