Pulsatile Tinnitus and Venous Sinus Stenosis
Overview
Pulsatile tinnitus is a rhythmic sound that often seems to match the heartbeat. Unlike common nonpulsatile tinnitus, pulsatile tinnitus may reflect an underlying vascular cause. One important cause is venous sinus stenosis, a narrowing in the brain’s venous drainage channels. Another important cause is a dural AV fistula, which must be considered because management differs and some fistulas carry significant risk.
The central task is to identify whether the sound is venous, arterial, structural, or related to abnormal shunting. Not every patient with pulsatile tinnitus needs a procedure, but the symptom should not be dismissed if it is persistent, clearly pulse-synchronous, or associated with neurologic, visual, or headache symptoms.
Common symptoms and associated issues
Patients may describe:
- Hearing a whooshing or heartbeat sound.
- Symptoms that change with head position or gentle neck pressure.
- Visual symptoms.
- Pressure-related symptoms, especially when intracranial venous outflow issues are involved.
Why the workup matters
Pulsatile tinnitus is a symptom, not a final diagnosis. The workup may involve:
- ENT evaluation in some cases.
- MRI/MRA or CT/CTA.
- MRV or CTV to assess venous anatomy.
- Review for signs of venous sinus stenosis, idiopathic intracranial hypertension, or dural AV fistula.
- Diagnostic cerebral angiography and venography in selected patients when noninvasive imaging is incomplete or when intervention is being considered.
When venous sinus stenosis may be relevant
Venous sinus stenosis refers to narrowing of a dural venous sinus, often the transverse or sigmoid sinus. In selected patients, that narrowing can contribute to altered venous flow dynamics and symptoms such as pulsatile tinnitus, headaches, or signs of raised intracranial pressure.
Treatment options
Treatment depends on the cause.
Option 1: Observation and workup completion
If symptoms are mild or the diagnosis remains uncertain, observation and further evaluation may be appropriate before any intervention is considered.
Option 2: Treat the underlying lesion
If the real cause is a dural AV fistula, aneurysmal issue, or other vascular abnormality, treatment should target that lesion rather than the symptom label alone.
Option 3: Venous sinus stenting
In carefully selected patients with symptomatic venous sinus stenosis, venous sinus stenting can be a minimally invasive treatment option. Through a catheter-based approach, a stent is placed across the narrowed venous segment to improve outflow and reduce abnormal flow turbulence or pressure gradients.
This option is generally considered only after careful imaging review and, when appropriate, physiologic confirmation that the stenosis is likely clinically meaningful.
Why patient selection matters
Not every narrowing seen on imaging needs a stent. The central question is whether the stenosis is truly causing the symptoms and whether stenting is likely to improve those symptoms with acceptable risk. A careful neurovascular evaluation helps separate incidental imaging findings from meaningful treatment targets.
For patients and families
Pulsatile tinnitus can be an identifiable and treatable vascular cause in some patients. A structured evaluation is important because the treatment should be directed at the actual source of the abnormal sound, whether that is venous sinus narrowing, a dural fistula, or another vascular issue.
